Administration Home visits Patients are encouraged to come to the surgery. Home visits are for patients who are too ill, infirm or severely disabled to come to the surgery. Home visits are very time-consuming and you will be seen more quickly and necessary investigations and treatments will be easy to carry out, if you can come to the surgery. If the patient has a temperature or rash, coming to the surgery would do no harm, and arrangements can be made to isolate infectious cases. If you require a home visit, please telephone 01276 857117 between 8:15am and 10 am. Unfortunately, with home visits, unlike surgery attendances, we cannot guarantee you will be seen by a particular doctor. Emergencies In case of emergency when the surgery is closed, you should always ring 01276 857117. You will be answered by a taped message (please have a pen and paper ready) giving the telephone number of the doctors cooperative on-call. Please listen to the whole of the taped message Repeat prescriptions If you are on regular treatment, your doctor may authorise repeat prescriptions for you. Please hand your written request, or computerised list, into reception at least 48 hours before you need the medication. If you would like us to return it by post, please enclose a stamped, addressed envelope. To avoid mistakes, we are unable to accept requests by telephone. As the surgeries are closed from 6:45pm Friday, prescriptions handed in after midday on Friday will take longer than 48 hours. Please note: we need to periodically review patients on regular medication. Therefore you may receive a slip attached to your prescriptions asking you to make an appointment to see your doctor. Test results If you need to ring the surgery to check test results, please ring after 2pm when the telephones are less busy and the receptionists have time to find the results for you (if they are available). Change of name or personal details In order to provide an efficient service, we need to have your personal details up-to-date, so please inform us of any changes of name, address or telephone number as soon as they occur. New patients Newly registered patients will be asked to complete a lifestyle questionnaire and have a health check with a doctor or practice nurse within one month of registering. We recommend you take advantage of this opportunity, since we are able to obtain an accurate record of your personal and medical details, and to inform you about the practice. Confidentiality All patient records on file or in the computer are strictly confidential and can only be released to a third party (e.g. insurance company) by the patients written consent. All information is covered under the data protection act. Children over the age of 16 are treated as adults and their medical information and test results will not be divulged to their parents without their consent. Results of laboratory tests and x-rays will only be given to patients themselves or to the parents of minors if appropriate. Compliments/complaints The practice has an in-house complaints procedure. We feel strongly that if any patient has a problem with a service we offer, we would like to know directly, so that the problem can be solved quickly, fairly and effectively. We welcome any suggestions for the improvement of the services we provide to our patients. Any compliments or complaints should be put in writing and addressed to the practice manager.
